SUMMER FORMS:
30 DAY MAILING
Summer Campers, below you will find two PDF Documents that make up the 30 Day Mailing. Just as with the 90 Day PDFs below, you do not need to print these out unless you absolutely need them. Please make sure to go through these two documents as well as all the documents under the 90 Day Mailing heading.
90 DAY MAILING
Summer Campers, below you will find the final six PDF documents mentioned and listed in your 90 Day Mailing. Please be sure to carefully read through each document. The only documents that you may need to print are as follows: Health History Form, Kid’s Age Group Form, and Waiver Form.
FINAL REGISTRATION INFORMATION
The Health History Form is required for kids 18 years old and under.
The Kid’s Age Group Form is only needed if your child wants to move up or down an age group.
The Waiver Form is required for all summer camp families. If you have multiple families registered under one name, please have each individual family fill out a waiver form. If possible, please print this document on one page, front and back for easier filing and organization. This multi-purpose form will help reduce and even elimnate time spent reading and signing waivers while at camp!

If you are renting a golf cart this summer, please read over these rules and regulations.
